Your Privacy

The privacy of your health information is very important to us. We want to keep your records private.

Health Choice Insurance Co. has policies in place to ensure your privacy.

To help ensure your privacy, we verify all incoming calls. Our Member Services staff will only talk to you or an authorized person about your care. This may be someone you have designated as a person we can talk to about your health care. Some examples might include a family Member or a close friend. If we are unsure about your wishes, we will not share any information with the caller.

To avoid problems, please tell Health Choice Insurance Co. Member Services about people we may talk to about your care. This may include family Members or friends. We will make sure our records include all of the people you want us to talk to about your care.

If you have legal papers showing you can act on behalf of a Health Choice Insurance Co. Member, please tell us. We need copies of court orders or papers. This includes legal guardians or people with Medical Power of Attorney (POA). These papers let us know it is OK to speak with you about that Member.

Our privacy notice is included in your new Member packet. The notice explains how we use your personal health information. It also explains how we protect your privacy. The notice tells you who we share your health information with. It also tells you when we will share your health information. If you would like another copy of this notice, please call Member Services. We will send you a copy at no cost to you.
